- Develop personalized nutrition care plans tailored to individual needs, incorporating evidence-based dietary recommendations and therapeutic interventions.
- Provide nutrition counselling and education to patients and their families, promoting healthy eating habits, disease prevention, and management.
- Collaborate closely with multidisciplinary healthcare teams, including physicians, nurses, and therapists, to ensure integrated care and optimal patient outcomes.
- Monitor and evaluate patients' progress, adjusting nutrition interventions as necessary to achieve treatment goals and improve health outcomes.
- Maintain accurate and detailed documentation of patient assessments, interventions, and outcomes in compliance with professional standards and regulatory requirements.
- Bachelor's or master's degree in Dietetics or Nutrition from an accredited institution.
- Registration with the Health and Care Professions Council (HCPC) as a practising Dietitian.
- Proven experience working as a Dietitian in a healthcare or clinical setting, with expertise in providing dietary counselling and education.
- Strong interpersonal and communication skills, with the ability to build rapport with patients and collaborate effectively with healthcare teams.
